Comparison of surgical outcomes in single-port laparoscopy with sentinel lymph node biopsy in enhanced recovery after surgery (ERAS) protocol versus conventional laparoscopy surgery for endometrial cancer.

- Abstract
Objective: This study compared the surgical outcomes between patients who underwent single-port laparoscopy with sentinel lymph node biopsy in enhanced recovery after surgery (ERAS) program versus conventional laparoscopy with lymph node dissection for endometrial cancer. Methods: We retrospectively reviewed the data of patients with endometrial cancer who underwent either single-port laparoscopic surgery or conventional laparoscopic surgery from January 2021 to July 2023 in single institution, Yonsei Cancer Center. We evaluated intra- and postoperative outcomes, progression-free survival, overall survival (OS), and recurrence rate. Results: A total of 173 patients, including those undergoing single-port laparoscopy with sentinel lymph node biopsy in ERAS program (n=45) and conventional laparoscopy with lymph node dissection (n=128), were analyzed. There were no significant differences in survival: 3-year disease-free survival was 95.6% versus 91.4% (p=0.497) and OS was 100% versus 98.4% (p=0.4) in single-port laparoscopy with sentinel lymph node biopsy in ERAS protocol and conventional laparoscopic surgery cohorts, respectively. The single-port laparoscopy with sentinel lymph node biopsy in ERAS protocol group had the longer operative time (205.0±56.3 minutes) but the less blood loss (47.3±90.9 mL) and the shorter postoperative hospital stay (2.0±0.9 days). In contrast, the conventional laparoscopic surgery group had the shorter operative time (133.6±56.3 minutes) but the more blood loss (55.5±69.8 mL) and the longer postoperative hospital stay (4.6±2.4 days). Conclusion: Single-port laparoscopic surgery with sentinel lymph node sampling in ERAS protocol for endometrial cancer demonstrates comparable survival outcomes and quick recovery.
