Objective: To systematically integrate barriers in applying "patient and family-centered care" in the adult intensive care unit (ICU). Methods: Relative literature that met the inclusion criteria was retrieved and analyzed from PubMed, The Cochrane Library, Web of Science, EMbase, CINAHL, Scopus, CNKI, Wanfang Database, VIP, and Sinomed database. Results: A total of 15 studies were included. The obstacle factors included 4 categories(28 types): medical staff factors(5 types), family factors(10 types), organizational factors(6 types), and patient factors(7 types). Conclusions: "Patient - and family - centered care" was affected by various barriers. The improvement strategies should be developed based on the modifiable barriers to promote the application of this measure in adult ICU.
