- Abstract
Objective:To explore the relationship between nutritional status and sleep time of surgery patients with head and neck cancer during hospitalization. Methods:Nutritional status and sleep time of 206 inpatients with head and neck cancer surgery in a tertiary grade A hospital in Shanghai were investigated by using bioimpedance-based body composition analyzer,patient-reported method and self-designed general patient questionnaire. Results:Body weight, body mass index(BMI),skeletal muscle mass and protein mass had gradually decreased during the hospitalization of patients with head and neck cancer, while the fat content had gradually increased. Body weight, BMI,skeletal muscle mass, and protein quality all had significantly decreased in the first week after operation(P=0.000). Body weight, BMI,and protein quality still had decreased apparently in the second week after operation(P<0.05). Fat content had an evident increasion in the first week after surgery(P=0.041). The daily sleep time before surgery was 6. 86±1. 79 hours. The daily sleep time in the first week after surgery was 3.73±1.74 hours. And the daily sleep time in the second week after surgery was 4.90±1.73 hours. Differences of sleep time at different times after surgery was statistically significant(P<0.05). There is an interaction between nutritional status and sleep time. Daily sleep time less than 6 hours in the first week after surgery caused a significant decrease in fat-free body mass and skeletal muscle mass in the first week after surgery. Weight and BMI in the second week after surgery had a significant decline,at the same time, fat content increased evidently. Patients with less muscle mass before and 1 week after surgery had less increase in sleep time in the second week after operation. In addition, the lower the fat content in the first week after operation was,the longer the sleep time was. Conclusions:Nutritional status and sleep problems of patients with head and neck cancer should be paid attention by medical staff, especially in the first week after operation. It is necessary to strengthen the dynamic evaluation and the joint management of nutrition and sleep,in order to increase the sleeping time and muscle mass, and decrease fat content, and finally to achieve rapid recovery and optimal patient outcomes.
