We report the case of a patient with infectious mononucleosis who presented with genital ulceration in an attempt to illustrate that EBV infection should always be considered in the differential diagnosis of vulvar ulcers in adolescence. A recent systematic review of the literature by Vismara et al. ([11]) described 158 patients and an infectious disease was present in 88% of them EBV virus infection (38 patients) followed by mycoplasma species (11 patients) were the main causes. The Epstein-Barr virus (EBV) causes infectious mononucleosis, one of the commonest viral infections in humans (Masucci and Ernberg [5]). Acute vulvar ulceration in children and non-sexually active adolescents is rare and presents as single or multiple painful ulcers with necrotic margins that usually resolve without scarring.
