This article discusses a case of delayed stroke following deep brain stimulation (DBS) placement in a 59-year-old female patient with tardive dyskinesia. DBS is a common neurosurgical treatment for movement and neuropsychiatric disorders. While complications following DBS placement are known, delayed stroke has not been previously reported. The patient had multiple risk factors for stroke, including hypertension, diabetes, and hyperlipidemia. The article highlights the importance of careful planning, pre-operative medical optimization, and avoiding hemodynamic fluctuations to reduce the risk of post-operative DBS-associated stroke.
