Abstract

The article is a response to a letter from Dr. Zimmern regarding the long-term efficacy of complete trigonal electrofulguration for women with recurrent urinary tract infections (RUTIs). The authors discuss the importance of selectively fulgurating external mucosa of the ureteral meatuses when lesions are present, without resecting the meatuses. They also mention the use of transabdominal control ultrasound and transvaginal ultrasonography of the bladder to monitor post-fulguration outcomes. The authors highlight the prevalence of trigonal squamous metaplasia in women and the potential hormonal influence on this condition. They apologize for an inaccuracy in their previous mention of the anesthesia used in another study and emphasize the use of the roller-ball method for cauterizing the entire trigone and bladder neck. The authors discuss the association between extensive lesions, histological metaplasia, and extra trigonal involvement with a greater chance of UTI recurrence. They also mention the detection of a low-grade urothelial tumor in one patient. The authors conclude by stating that longer follow-up periods will be covered in future publications.
