- Abstract
Leitsymptome zur Gallenwegsdiagnostik sind der Oberbauchschmerz, der Ikterus oder die laborchemisch manifeste Cholestase. Zusammen mit ihren Komplikationen ist die Cholelithiasis in der Mehrzahl der Fälle die Ursache entzündlicher Erkrankungen von Gallenblase und Gallenwegen. Dabei wird zwischen akuten und chronischen Formen unterschieden. Bei den chronischen Formen der Gallenwegsentzündung sind die primär sklerosierende Cholangitis (PSC), die primär biliäre Zirrhose (PBC) sowie die IgG4-assozierte sklerosierende Cholangitis von besonderer Bedeutung. Neben gutartigen und „tumor-like lesions" sind als maligne Tumoren das Gallenblasenkarzinom und das cholangiozelluläre Karzinom (CCC) in seinen 3 Höhen zu differenzieren. Trotz aller Verbesserungen ist die Spezifität der Bildgebung bei den Malignomen weiterhin eingeschränkt, sodass die endgültige Klärung häufig der histologischen Aufarbeitung vorbehalten bleibt.
