- Abstract
Objetivo: caracterizar la hiperostosis asociada a meningiomas (MNGs) de pacientes atendidos en el Instituto Nacional de Neurología y Neurocirugía Manuel Velasco Suárez (INNN). Antecedentes: los MNGs son los tumores primarios más frecuentes del sistema nervioso central. Proceden de las cubiertas de cerebro y médula espinal, causando alteraciones neurológicas diversas. En el 46 al 72% de los casos se asocian a hiperostosis. No hay estudios sobre la frecuencia y características de hiperostosis asociada a MNGs en población mexicana. Métodos: se estudiaron 189 pacientes con MNGs, reclutados de 2017 a 2019 en el INNN. Se analizó edad, sexo y sintomatología en los pacientes y localización anatómica, subtipo histológico, grado de malignidad y presencia de hiperostosis en los tumores. Se determinó frecuencia porcentual de: los MNGs por edad y sexo; de los síntomas; y de hiperostosis en relación a la localización de los tumores. Núm. de registro del protocolo: 47/20 Resultados: los MNGs fueron más frecuentes en mujeres que en hombres (65 y 35%, respectivamente). El grupo de edad con mayor proporción de tumores fue el de 45 a 54 años. Los síntomas más frecuentes fueron: cefalea (60%), movimientos anormales (41%), alteraciones visuales (20%), crisis convulsivas (13%) y pérdida de la audición (13%). Las localizaciones anatómicas más frecuentes de los MNGs fueron: frontal (29%), temporal (17%) y parietal (16%); y los subtipos histológicos predominantes, fibroblástico (25.7%), meningotelial (21.8%) y transicional (20%). El grado de malignidad de los MNG fue en orden decreciente de frecuencia : Grado I (73%); Grado II (24%) y Grado III (3%). Treinta y dos de 189 pacientes (17%) presentaron hiperostosis. Veinte de estos últimos, se localizaron en el área esfenoidal (63%); 7, en área frontoorbital (22%); y 5, en la convexidad (15%) (Figura 1). Conclusiones: los MNGs son más frecuentes en mujeres que en hombres, predominan en la quinta y sexta décadas de la vida y los síntomas más frecuentes que ocasionan son cefalea, movimientos anormales y alteraciones visuales. Su localización más frecuente es frontal. Sus grados de malignidad preponderantes son GI y GII. El 17% de los MNGs en pacientes mexicanos desarrolla hiperostosis, asociada fundamentalmente a las sublocalizaciones esfenoidal, fronto-orbitaria y del área de la convexidad.
