Alteraciones de sueño en adultos mayores con y sin demencia Alzheimer durante cuarentena por COVID-19.

- Abstract
Objetivo: describir las alteraciones de sueño de insomnio y síndrome de apnea e hipoanea obstructiva del sueño (SAHOS) en adultos mayores con y sin demencia Alzheimer, durante la cuarentena. Antecedentes: el sueño es una función vital que cambia durante la vida, siendo vulnerable al envejecimiento, factores ambientales y patologías neurodegenerativas (Stern & Naido., 2015), siendo el insomnio y el SAHOS unas de las más prevalentes en esta población (Morgan, 2003; Borges et al., 2019), El metaánalisis de Sánchez- García, et al (2022), reporta alteración en síntomas psicoconductuales en pacientes con y sin demencia durante la cuarentena por COVID-19, siendo la alteración de sueño una de ellas. Método: se utilizó el cuestionario de alteraciones de sueño en el adulto mayor, el PHQ-9, el test de MoCA y una entrevista estructurada para observar la presencia de síntomas de insomnio y SAHOS en dos grupos de adultos mayores sin demencia (SD; n=31) y con demencia Alzheimer (CD; n=31). El protocolo se aplicó a pacientes y cuidadores. La evaluación se realizó durante el segundo semestre de 2021, periodo en el cual se reanudaba consulta externa y los pacientes y cuidadores reportaban quejas de alteración de sueño durante la época de pandemia. Núm. de registro del protocolo: 11/20 Resultados: en el grupo SD la edad promedio fue 69,4 años y principalmente de género femenino (56,7%). En el grupo CD la edad media fue de 72 años y el género femenino fue el prevalente con un 51,6%. En cuanto a SAHOS, la somnolencia diurna, los ronquidos y sueño poco reparador se manifestaron en ambos grupos, siendo mayor en CD. En cuanto a insomnio, en el grupo CD presentó dificultad para conciliar el sueño, cambios en el estado del ánimo e insatisfacción con dormir; en el grupo SD se encuentra despertar nocturno, conciliación, olvidos y problemas atencionales. Se encontró asociación en ambos grupos con depresión e insomnio (p=0.00) y SAHOS (p=0.017), con una asociación inversa entre el puntaje del MoCA y los problemas de sueño (p=0.00). Conclusiones: los síntomas de SAHOS, se presentaron de forma similar en ambos grupos. En cuanto al insomnio, solo los problemas de conciliación se encontraron en CD y SD. En general todos los problemas de sueño se presentaron con mayor frecuencia en CD. Factores como la edad, depresión y un mejor desempeño cognitivo, mediaron la presencia de las alteraciones. Se debe realizar seguimiento de esta sintomatología postpandemia y conocer si existe un cambio.
