- Abstract
Clinical trials have shown that beta-blockers should not be the first line agent for the treatment of uncomplicated arterial hypertension, irrespective of the heart rate. Beta-blockers reduce blood pressure compared with placebo, but compared with other antihypertensive agents, the blood pressure-lowering efficacy is suboptimal; beta-blockers have a lesser effect on the more important central aortic pressure when compared with reninangiotensin system blockers, diuretics, and calcium antagonists. Also, after three decades of using betablockers for hypertension, no study has shown that beta-blocker monotherapy reduces morbidity and mortality in hypertensive patients, even when compared with placebo. When arterial hypertension has complications or is associated with other cardiovascular diseases, the use of beta-blockers is indicated. For a patient on beta-blocker treatment, epinephrine can have different effects at the dentist consulting room. Selective betablockers, used mostly now, do not determine hypertensive reactions or resistance to anaphylaxis treatment, unlike non-selective beta-blockers. However caution is necessary when using epinephrine, whether the type of the beta-blocker is selective or non-selective.
