This study objective is to analyze the difficulties involved in the implementation of safety surgery protocols in a public hospital in Rio de Janeiro by their employees view. The method applied was a qualitative approach based on in-depth interviews, based in a semi-structured script developed by the academic literature about this theme. Were interviewed 15 professionals working in the operation room of Miguel Couto's Municipal Hospital, located in Rio de Janeiro city (RJ), falling into five groups: surgeons, anesthesiologists, medical residents, nurses and nurses assistants. The study showed diverse opinions surrounding three aspects: lack of effective ways of penalty, reduced number of nursing and technical nursing professionals and lack of training of the involved professionals - the latter a converging aspect with the literature about this theme. The conclusion is that singular aspects of Brazilian public health system are additional obstacles for those already known by the literature about the theme.
