- Abstract
Amaç: Ailevi Akdeniz ateşi (FİVIF) MF.FV gen mutasyonu ile ilişkili olup, en sık görülen otoinflamatuvar hastalıktır. Hastalık serozit ve artritin intemıittan febril atakları ile karakterize olmasına karşın bazı hastalarda komplikasyon gelişebilmektedir. İnfertilite/subfertilite otoinflamatuvar hastalıkların komplikasyonları olarak görülmekle birlikte, FİVIF ilişkili infertilite ile ilgili yeterli veri bulunmamaktadır. Bu çalışmanın amacı FıVLF'e bağlı infertilite/subfertilite sıkkğım ve ilişkili faktörleri araştırmaktır. Yöntem: Çalışmaya FİVIF in Central Anatolia (FICA) kohor-tundaki çocuk istenü olan 403 erişkin hasta (ortalama yaş 41.4±10.7, %68.7 kadın) dahil edildi. Bütün hastalar Tel Has-homer krieterlerini karşılamakta olup en az 1 yıllık kolşisin kullanım öyküsü mevcuttu. Demografik özellikler, FİVIF atakları, gen mutasyon analizleri, amiloidoz ve alınan tedaviler kaydedildi. Bulgular: FMF atak oranlan %89.6 peritonit, % 83.1 ateş, %51.4 plevrit, %43.4 artrit ve %28 raş şeklindeydi. MEFV nıu-tasyonu 317 kişide çalışılnuş olup, hastaların %77.9'uııda M694V mutasyonu (%27.4 homozigot) mevcuttu. 42 (%10.4) hasta kol-şisin yanıtsızdı. İnfertilite 45 hastada (%11.2) mevcuttu. Bunların 38'i erkek, 7'si kadındı. Kadın cinsiyet (OR: 2.9; %95 CI, 1.07-7.98) ve kolşisin yanıtsızlığı (OR: 2.9; %95 CI, 1.07-7.98) infertilitenin bağımsız risk faktörleri olarak belirlendi. Sonuç: Kadın cinsiyet ve kolşisin yanıtsızlığı infertilitenin bağımsız risk faktörleri olarak belirlendi. Etkili tedavi yöntemleriyle bu hastalarda fertilite oranı artabilir.
