- Abstract
Introduction: Lipoid pneumonia is a rare disorder determined by the accumulation of animal, vegetal or mineral oil in pulmonary alveoli. This accumulation produces an acute and chronic inflammation in the lung parenchyma. Case report: We present a 38-year-old woman report with a challenging respiratory pathology. She is a heavy smoker and presented to the family doctor with fever, chills, myalgia, cough with sputum and chest pain. The physical examination showed signs of inferior left pneumonia. Chest x-ray revealed a persistent opacity in the inferior pulmonary left lobe. The specific bacterial etiology, defined by specific tests in the Infectious Diseases department, was negative. One-month follow-up showed the same X-ray image, hence more examinations were considered. Thoracic ultrasonography and CT scan, and bronchoscopy helped the differential diagnosis of lung tumor. As the results were still inconclusive the pneumologist proposed the Oil Red O test which confirmed the lipoid pneumonia. The case management includes smoking cessation, prevention of oil inhalation, corticosteroid anti-inflammatory treatment and recommendation for flu and pneumococcal vaccine. Conclusion: The family doctor should think about this etiology of pneumonia, especially when the patient is working with or using oily substances and prescribe antibiotics only if the pneumonia is over infected.
