Langzeitnachsorge nach allogener Stammzelltransplantation.

- Abstract
Hintergrund: In Deutschland erhalten jährlich über 3000 Patienten eine allogene hämatopoetische Stammzelltransplantation (alloHSZT). Aufgrund von Fortschritten in der Therapie überleben viele Patienten zwar länger als früher, aber haben daher ein erhöhtes Risiko für Langzeitkomplikationen. Diese können die Lebensqualität erheblich beeinflussen. Methode: Auf Basis einer selektiven Literaturrecherche in PubMed wurde eine Literaturübersicht zu den Langzeitfolgen nach alloHSZT erstellt. Dabei wurden Einzelfallberichte ausgeschlossen und internationale Leitlinien miteinbezogen. Ergebnisse: Es gibt kaum randomisierte Studien zur Langzeitnachsorge nach alloHSZT, allerdings existieren international konsentierte Leitlinien. Zehn Jahre nach alloHSZT entwickeln 50-70% der Patienten eine chronische Graft-versus- Host-Erkrankung (cGVHD). Da Transplantatempfänger ein erhöhtes Infektionsrisiko, inklusive der Reaktivierung von endogenen Herpesviren, haben, wird die leitliniengerechte Durchführung von Impfungen empfohlen. Eine gonadale Dysfunktion, die unter anderem vom Transplantationszeitpunkt und der Radiotherapie abhängig ist, tritt bei bis zu 92% der Männer sowie 99% der Frauen auf. Die einzunehmenden Medikamente oder eine transfusionsassoziierte Hämosiderose können die Leberfunktion beispielsweise beeinträchtigen. 40-50% der Patienten leiden an Störungen des Fettstoffwechsels, die mit Herzinfarkt, peripherer arterieller Verschlusskrankheit und Schlaganfall assoziiert sind. Die Lebenserwartung ist im Vergleich zur Gesamtbevölkerung vermindert Schlussfolgerung: Neben Präventivmaßnahmen ist eine individualisierte, risikoadaptierte und multidisziplinäre Nachsorge der Patienten erforderlich, um Langzeitfolgen korrekt zu diagnostizieren und adäquat zu behandeln. Prospektive Studien zu den Spätfolgen, aber auch eine konsequente Dokumentation von Langzeitfolgen in überregionalen Datenbankregistern können die Langzeitnachsorge verbessern.
