- Abstract
Objective: Electrocardiography parameters including Tpeak‐to‐Tend interval, Tpeak‐to‐Tend/QTc ratio, and QRS‐T angle have gained importance for predicting ventricular arrhythmia in recent years. Ventricular arrhythmias have a high risk of cardiac death in patients with low systolic functions. We aimed to evaluate the association between the Tpeak‐to‐Tend interval, Tpeak‐to‐Tend/QTc ratio, QRS‐ T angle, and ventricular arrhythmia risk detected by implantable cardioverter‐defibrillator. Material and Method: Thirty‐four patients (mean age was 69.26±11.81) who had low ejection fraction and undergone implantable car‐ dioverter‐defibrillator implantation for primary prophylaxis of sudden cardiac death were included prospectively. Tpeak‐to‐Tend inter‐ val, Tpeak‐to‐Tend/QTc ratio, and QRS‐T angle were analyzed from 12 lead electrocardiography at the same time with implantable car‐ dioverter‐defibrillator checking. Patients were divided into two groups according to ventricular tachycardia detection during imp ‐ lantable cardioverter‐defibrillator checking; ventricular tachycardia group and or non‐ventricular tachycardia group. Results: Tpeak‐to‐Tend/QTc ratio in lead V4, V5, V6 was higher in ventricular tachycardia group (20.06±4.08 vs 15.21±2.61, p=0.002; 20.09±2.83 vs 15.41±2.36, p<0.001; 18.39 ± 2.61 vs 14.58±1.81, p<0.001; respectively). As compared QRS‐T angle, the ventricular tachy‐ cardia group had an abnormally widened QRS‐T angle than the non‐ventricular tachycardia group (p=0.024). Among demographic fea‐ tures and electrocardiography findings, Tp‐Te/QTc ratio ‐lead V5‐ was the only independent predictor of ventricular tachycardia in mul‐ tivariate regression analysis (OR=3.161, 95%CI=4.240‐2.357, p=0.007). Conclusion: Prolonged Tpeak‐to‐Tend interval and Tpeak‐to‐Tend/QTc ratio might be used to predict ventricular tachycardia incidence detected by implantable cardioverter‐defibrillator. Widened QRS‐T angle is associated with the incidence of ventricular tachycardia in patients with implantable cardioverter‐defibrillator.
