- Abstract
Stroke is a medical emergency that can result in severe brain damage and significant functional disability. Accurate clinical evaluation plays a crucial role in determining the type of stroke and in choosing the appropriate therapeutic interventions. In cases of ischemic stroke, endovascular thrombectomy emerged as an effective intervention to remove the clot responsible for arterial obstruction. Objective: The objective of this systematic review is to examine the available scientific literature on the clinical evaluation of stroke and the indication of endovascular thrombectomy as a therapeutic approach. The review aims to understand the criteria used in the clinical evaluation of the stroke, identify the patients who would benefit from thrombectomy and evaluate the results related to this intervention. Methodology: The systematic review adopted the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyzes) guidelines. The PubMed, Scielo and Web of Science databases were consulted to identify relevant studies. Descriptors used included: stroke, clinical assessment, thrombectomy, endovascular treatment and interventional neuroradiology. Inclusion Criteria: Studies published in the last 10 years; focus on clinical evaluation of patients with stroke; investigation of indication and results of endovascular thrombectomy; access to full text in English, Portuguese or Spanish and studies that reported the efficacy and safety of thrombectomy. Exclusion Criteria: Studies not related to clinical evaluation of stroke or endovascular thrombectomy and studies in languages other than those mentioned above. Results: 15 articles were selected. The reviewed literature emphasized the importance of rapid and accurate clinical evaluation to determine the viability of endovascular thrombectomy. The studies identified criteria for selecting patients, such as the time since the onset of symptoms, the size of the heart attack and the location of vascular obstruction. In addition, the review highlighted positive results reported with thrombectomy, including better recanalization rates, reduced functional disability, and better prognosis compared to patients treated with thrombolytic therapy alone. Conclusion: The rigorous clinical evaluation plays a central role in the identification of patients who are candidates for endovascular thrombectomy as part of the treatment of ischemic stroke. The systematic review showed that well-defined criteria for patient selection and rapid implementation of thrombectomy are crucial to improve clinical outcomes. Understanding the benefits of this therapeutic approach underscores the importance of collaboration among health care professionals to ensure proper treatment and optimize the outcomes of stroke patients.
