- Abstract
INTRODUCCIÓN: El prolapso de órganos pélvicos, es una condición común que afecta a cerca de la mitad de las mujeres. Se relaciona con el incremento de la edad, así como con la histerectomía, siendo de esta, la principal complicación a largo plazo, ocurriendo aproximadamente entre el 0,2-43% de los casos. Se estima que una de cada 9 mujeres se le realizará histerectomía en el transcurso de su vida, y en el 10% de estas mujeres, se requerirá reparación quirúrgica. Los objetivos de cualquier manejo quirúrgico del prolapso vaginal es restaurar la anatomía vaginal, y mantener la función sexual y la durabilidad. La colposacropexia Se considera como el estándar de oro para el manejo del prolapso vaginal apical, debido a que preserva el eje normal de la vagina, la profundidad máxima de la misma, y el uso de material sintético provee la fuerza suficiente en pacientes con tejido nativo débil, por lo que es superior a la cirugía vía vaginal. La tasa de éxito se ha reportado entre el 93-100% con este tipo de procedimiento quirúrgico, y del 74% de éxito a largo plazo. CASO CLÍNICO: Paciente femenina de 80 años de edad, con antecedentes de multiparidad (G8P8V8) e histerectomía hace 37 años por patología benigna, con cuadro de 8 años de evolución de sensación de masa en vagina, al realizar esfuerzos físicos, que se exacerba hace un año, con exteriorización de la misma. Al examen físico: Prolapso anterior grado II, Prolapso posterior grado II, Prolapso apical grado III-IV. CONCLUSIÓN: La cirugía laparoscópica para realización de sacrocolpopexia, combina las ventajas del procedimiento abierto, con las ventajas propias de los procedimientos de mínima invasión, con resultados clínicos comparables entre ambos procedimientos; a la vez que permite múltiples tratamientos para la disfunción del piso pélvico, como el prolapso uterino, el prolapso vaginal, el rectocele o el enterocele.
