- Abstract
Vermoeidheid is de belangrijkste klacht van (behandelde) Addisonpatiënten. Mogelijk hangt dit samen met een afwijkende catecholaminerespons. Twee Addisonpatiënten zonder (A) en met (B) moeheidsklachten ondergingen beide een maximale inspanningstest. In rust, op de anaerobe drempel en tijdens maximale inspanning werd de adrenaline-, noradrenaline- en lactaatconcentratie bepaald. De adrenalinerespons was afwezig in beide patiënten. De noradrenalinerespons was in patiënte A zeer uitgesproken vergeleken met patiënte B (6217 vs. 1070 pg/ml). Er was geen duidelijk verschil in lactaatspiegels. Patiënte B kreeg een proefbehandeling met een b2- agonist (salbutamol 300 mcg subcutaan) waarop zij een sterke orthostatische hypotensie ontwikkelde. Deze case studie suggereert dat moeheid bij Addisonpatiënten mogelijk wordt veroorzaakt door een relatief geringe noradrenalinerespons tijdens inspanning. Indien noradrenaline het adrenalinetekort zou kunnen compenseren, dan kan een b2-agonist een zinvolle aanvulling van de suppletietherapie zijn. Deze uitbreiding van de behandeling bleek echter ongeschikt, doordat het een sterke orthostatische hypotensie veroorzaakte.
