- Abstract
Introducere. Este bine cunoscut faptul că incidența maximă a infecției cu HPV este în primii 5-10 ani de la debutul vieții sexuale. Totuși, femeile sunt expuse toată viața la contractarea virusului. Cu toate că este recomandat în primul rând adolescentelor, în 2018 vaccinul împotriva HPV a fost aprobat și pentru persoanele de până la 45 de ani. Deși leziunile cervicale sunt întâlnite din ce în ce mai frecvent la persoane tinere, majoritatea pacientelor cu displazie cervicală au vârste de peste 30 de ani. Ne propunem să aflăm dacă vaccinul se adresează cazurilor cu displazie diagnosticată, cu modificări ale examenului citologic, pentru prevenirea progresiei bolii până la stadiul de neoplazie intraepitelială. Materiale și metodă. Utilizând baze de date internaționale (Cochrane Central Register of Controlled Clinical Trials, PubMed, UpToDate), am analizat lucrări publicate în literatura de specialitate până în prezent. Rezultate. Studii precum FUTURE II, VIVIANE ș.a. susțin eficiența imunizării împotriva HPV, numărul de cazuri de displazie până la neoplazie intraepitelială fiind redus semnificativ, inclusiv la paciente care au beneficiat de vaccin după vârsta de 26 de ani. Concluzii. Deși în cazul infecției naturale cu HPV răspunsul imun declanșat este slab, primele studii efectuate arată că imunitatea dobândită prin vaccinare se menține și protejează până la 10 ani împotriva celor mai importante și agresive tulpini.
