All patients presented as von Zumbusch of GPP, including four paediatric GPP and two IH patients. Patient 10, diagnosed as GPP with Acrodermatitis continua of Hallopeau, did not achieve IGA 0 on 6-month secukinumab treatment for his recalcitrant digital lesions. Dear Editor: Generalized pustular psoriasis (GPP) is a rare and refractory variant of psoriasis, which is characterized by relapsing visible and neutrophil-rich sterile pustules on non-acral skin.[1] The condition was classified into acute GPP of von Zumbusch, annular pustular psoriasis, localized GPP and exanthematic GPP.[2] The variant characterized by a generalized pustular eruption anytime during pregnancy is also designated as impetigo herpetiformis (IH), and the juvenile or infantile variant could also be called paediatric GPP.[3] Previous studies have verified the critical role of I IL-36RN i gene in the pathogenesis of GPP.[4] Functional activation of IL-36R closely related to TNF- /IL-23/IL-17 axis, forming a positive inflammatory loop.[5] Therefore, blocking IL-17 might be a promising therapy for the disease.[[6]] Here, we presented a series of acute GPP patients who showed rapid and sustained response to secukinumab.
