RESPUESTA INMUNOLÓGICA A LA VACUNACIÓN CONTRA SARS-COV2 EN PACIENTES CON MIASTENIA AUTOINMUNE.

- Abstract
Objetivo: Reportar la respuesta inmunológica a la vacunación contra SARS-CoV-2 en pacientes con Miastenia Autoinmune (MA) en tratamiento inmunosupresor. Antecedentes: MA es una enfermedad causada por la presencia de anticuerpos contra componentes de la unión neuromuscular, la mayoría de los casos dirigidos al receptor de acetil-colina 1. Tiene una incidencia de 8 a 10 casos por millón de personas y una prevalencia de 150 a 250 casos por millón de personas 2. La evidencia a lo largo de la pandemia por SARS-CoV-2 ha mostrado que las condiciones como neoplasias y enfermedades autoinmunes pueden disminuir la respuesta inmunológica durante la exposición al virus o a la vacuna. Los reportes sugieren que estos pacientes tienen una limitada formación de anticuerpos post-vacunación. Jama Neurology publicó un estudio en 112 pacientes, 49 tratados con Ocrelizumab en donde los pacientes tratados en los 6 meses previos a la vacunación formaron una baja cantidad de anticuerpos con respecto a los pacientes sanos y a los tratados con tiempo mayor a 6 meses9. Con lo anterior, surge la necesidad de investigar si los pacientes con diagnóstico de MA que se encuentran inmunocomprometidos por su esquema de tratamiento tienen una respuesta humoral adecuada a la vacunación contra SARS-CoV-2 comparado con pacientes MA sin tratamiento inmunosupresor. Aún no se han creado lineamientos basados en evidencia para la vacunación en pacientes con MA. Métodos: Estudio de casos y controles anidado en una cohorte de pacientes con MG. Se utilizó una muestra de 82 pacientes con MA. Catorce sin tratamiento inmunosupresor y 68 en tratamiento inmunosupresor. Resultados: De los 82 pacientes estudiados, 62.2% son del genero femenino y 37.8% masculino. La mayoría, hasta un 92.7%, residen en la zona centro de México. En cuanto al tratamiento, 93.9% de los pacientes toma piridostigmina; 81.7% están en tratamiento con corticoesteroide (prednisona); 64.6% de los pacientes toma inmunosupresor, principalmente Azatioprina (58.5%) y Rituximab (35.4%). El 36.6% de los casos reportaron haber cursado con un cuadro de infección por SARS-CoV-2 y la mayoría de todos los casos, hasta un 92.7%, recibieron al menos dos dosis de vacuna contra SARS-CoV-2, 98.8% de los pacientes tuvieron títulos positivos de anticuerpos anti-S positivos y solo 69.5% tuvieron títulos positivos de anticuerpos anti-N. Conclusiones: Los pacientes con MA en inmunosupresión tienen formación de anticuerpos postvacunación.
