Desempeño de tareas visoespaciales en pacientes con déficits cognitivos subjetivos posterior a COVID-19.

- Abstract
Objetivo: valorar la ejecución en las pruebas visoespaciales en pacientes con queja cognitiva después de haber presentado COVID-19. Antecedentes: la creciente evidencia sugiere que los pacientes con síndrome COVID post-agudo, con frecuencia informan quejas cognitivas [Hewitt KC, 2021). Por lo que realizar una evaluación cognitiva es fundamental para establecer un diagnóstico objetivo en estos pacientes (Ahn HJ, 2010) y se destaca la utilización de pruebas visoespaciales como la prueba de dibujo de reloj y la Figura compleja de Rey-Osterrieth (FCRO) para predecir el deterioro cognitivo (Young Chul Youn 2021) Métodos: 53 pacientes diagnosticados con COVID-19 referidos del INER, y quienes reportaron queja cognitiva y obtuvieron puntaje menor a 26 en el MoCA, fueron evaluados tres meses después utilizando una batería neuropsicológica completa y específicamente para valorar el funcionamiento visoespacial, se aplicó la copia de FCRO, la copia de un cilindro y la prueba del reloj. La calificación de la FCRO se hizo cuantitativa y cualitativa de acuerdo a Sistema de puntuación cualitativa de Boston (BQSS). Se aplicó la Rho de Spearman para el análisis de los datos. Num. registro de protocolo: 0621 Resultados: el promedio de edad fue de 57.17 (±11.45) años y de escolaridad 10.89 (±4.31) años. Las pruebas visoespaciales mostraron correlaciones moderadas con el puntaje total del MoCA. La copia de FCRO (.281 p > 0.013), la copia del cilindro (.455 p > 0.001) y la prueba del reloj (.433 p > 0.001). El análisis de los componentes de la figura mostró correlación con los ítems que evalúan la función visoespacial y ejecutiva, estructura de la base (.495 p > 0.001), detalles con significado (.384 p > 0.004), detalles agrupados (.327 p > 0.017) y detalles lineales (.404 p > 0.003). Conclusiones: la correlación observada de las pruebas visoespaciales con el MoCA nos señala la sensibilidad de éstas en la identificación de deterioro. Por otro lado, el análisis cualitativo del desempeño de los pacientes en estas tareas visoespaciales, podría ser de gran relevancia para la identificación pacientes con diferentes patrones de déficits cognitivos, y por lo tanto podrían beneficiarse de enfoques preventivos y/o terapéuticos específicos.
