Clinical Controversies in Breast Cancer: A Thought Leader Forum and Consensus Roundtable.

- Abstract
The management of breast cancer has improved significantly resulting in prolonged survival and a better quality-of-life. However, many clinical situations relating to the diagnosis, treatment and follow up of patients still lack clear evidence. To provide medical oncologists with direction in this area, a consensus roundtable was convened in Berlin, Germany, prior to the 6th European Breast Cancer Conference in April 2008. Four common patient scenarios are presented with the relevant scientific evidence, and where such literature is not available, expert opinion is provided to uphold the following consensus by the panel. The roundtable concluded that there is indirect evidence to support the role of systemic treatment after suitable initial local therapy for local recurrence in early breast cancer (EBC). Current evidence does not yet provide a clinical basis for surgery for removal of primary tumors in metastatic breast cancer (MBC) except where local regional control is an issue. The use of positron emission tomography (PET) scanning as an initial staging tool following diagnosis of EBC is not supported by data, but may be considered in the metastatic setting for patients with rising tumor markers. For breast cancer patients who have relapsed on adjuvant endocrine therapy, switching to another hormonal treatment should be considered alongside the use of bisphosphonates and palliative treatment where appropriate. Consensus forums such as this generate valuable insights into the management of breast cancer in everyday clinical practice, and should be encouraged among clinicians with diverse skills and expertise.
