Obstrução esofágica total secundária a estenose severa em felino - Relato de caso.

- Abstract
Foi encaminhado para realização de endoscopia digestiva alta um paciente felino, fêmea, 8 anos, SRD, com histórico de polifagia, perda de peso e regurgitações frequentes com piora progressiva. Animal teria sido submetido, recentemente, a exérese cirúrgica de carcinoma espinocelular em orelha direita e recebido medicação oral, doxiciclina, por sete dias consecutivos como tratamento de hemoparasitose. Durante avaliação endoscópica notou-se completo fechamento de lúmen esofágico de porção torácica proximal por evolução de estenose de provável origem por refluxo trans-operatórios e medicamentoso com moderada dilação esofágica anterior e pequena estase de conteúdo alimentar. Por não haver lúmen em membrana fibrótica para passagem de balão esofágico pela estenose optou-se pela realização de uma pequena perfuração central com pinça de biopsia flexível de 1.8mm para criar um orifício de diâmetro suficiente para passagem do balão progressivo (10/11/12mm). Pós dilatação, foi instituído tratamento com antibioticoterapia, antiácido, pró-cinéticos, anti-inflamatório esteroidal e analgésicos. Indicou-se sessões semanais de balonamento esofágico por endoscopia. Na terceira sessão notou-se boa amplitude de anel esofágico com melhora no aspecto de mucosa esofágica e evolução positiva do quadro digestivo do paciente, já se alimentando com dieta semissólida e havendo ganho de peso. Pouco antes da realização do quarto procedimento endoscópico houve baixa evolução clinica no tratamento oncológico do paciente com significativa piora de exames hematológicos e tutor optou por eutanásia.
