Therapeutisches Management akraler Manifestationen der systemischen Sklerose.

Zusammenfassung??Zu den klinisch relevanten akralen Manifestationen der systemischen Sklerose (SSc) z?hlen das Raynaud-Ph?nomen, die Calcinosis cutis und die Sklerodaktylie. In fortgeschrittenen Stadien der Erkrankung kann es zu dermatogenen oder arthrogenen Kontrakturen und als Folge einer obliterativen Vaskulopathie zu digitalen Ulzerationen und ausgedehnten Nekrosen kommen. Die Behandlung sollte interdisziplin?r unter Mitwirkung von Rheumatologen, Dermatologen, Handchirurgen und Physiotherapeuten erfolgen und ggf. eine psychologische Begleitung einschlie?en.Bei der medikament?sen Therapie des SSc-assoziierten Raynaud-Ph?nomens erwiesen sich insbesondere Calciumantagonisten, der ?1-Rezeptoren-Blocker Prazosin, Prostacyclinanaloga sowie in Pilotstudien der Angiotensin-II-Antagonist Losartan und der Serotonin-Wiederaufnahmehemmer Fluoxetin als wirksam. Zur Behandlung isch?mischer akraler Ulzerationen sind in erster Linie parenterale Prostacyclinanaloga, z. B. Iloprost, empfehlenswert. Bei unzureichendem Effekt kommen Therapieversuche mit einem Phosphodiesterase-V-Inhibitor, z. B. Sildenafil, zur Ulkusabheilung und mit einem Endothelin-Rezeptor-Antagonisten, z. B. Bosentan, zur Pr?vention neuer Ulzerationen in Betracht. F?r die Therapie der Calcinosis cutis und der Dermatosklerose l?sst sich trotz Teilerfolgen einiger Substanzen f?r kein Pharmakon eine generelle Indikation ableiten.Chirurgische Therapieoptionen beinhalten die Exzision oder K?rettage von symptomatischen Kalzinosen und f?r die vaskul?re Problematik die digitale Sympathektomie, Arterienrekonstruktionen und in sehr seltenen Ausnahmef?llen Amputationen. Bei Kontrakturen k?nnen Arthrodesen der Fingermittelgelenke sowie Resektionsarthroplastiken oder prothetische Versorgungen im Bereich der Fingergrundgelenke die Greiffunktionen verbessern.
