- Abstract
Rana oparzeni owa jest narażona na kolonizację i zakażenie przez niemal każdy drobnoustrój. Szczególna podatność rany oparzeniowej na zakażenie jest wynikiem współdziałania wielu czynników - zarówno ze strony organizmu chorego, jak i otaczającego środowiska. Czynnikami szczególnie usposabiającymi do zakażeń rany oparzeniowej są: wiek pacjenta (dzieci i ludzie starzy częściej ulegają zakażeniom), głębokość oraz rozległość oparzenia, współobecność chorób przewlekłych (np. cukrzyca) oraz przedłużony odstęp czasu pomiędzy urazem termicznym a podjęciem skutecznego leczenia (przedłużona kontaminacja). Rana oparzeniowa nie goi się w sposób stabilny i często dochodzi do pogłębienia się martwicy. Jednym z najistotniejszych czynników miejscowych usposabiających do zakażenia jest zaburzony dopływ krwi do rany, powodowany koagulacją naczyń krwionośnych. Również objawy ogólne, takie jak obniżone ciśnienie tętnicze we wstrząsie poopa-rzeniowym, upośledzają ukrwienie rany. Objawy ogólne, takie jak: obniżenie stanu odporności chorego (szczególnie funkcji układu siateczkowo-śródbłonkowego), obniżenie aktywności opsoninowej, chemotaksji, fagocytozy oraz zdolności bakteriobójczych leukocytów, wpływają na pogorszenie gojenia rany. Także pewne działające wprost ze środowiska rany czynniki zwiększają ryzyko jej zakażenia. Należą do nich: niskie pH rany (chory z kwasicą), hipotermia, maceracja oraz obecność ciał obcych. Zarówno te zagadnienia, jak i zaburzenia w procesie naprawczym rany oparzeniowej będą przedmiotem wystąpienia.
