Morfología del hipocampo y distinción entre la depresión de inicio tardío y la depresión de inicio precoz en ancianos.

- Abstract
Objetivo: A pesar de las pruebas de la existencia de anomalías del hipocampo en la depresión en ancianos, se desconoce si estos cambios son regionalmente específicos. Este estudio empleó técnicas cartográficas en tres dimensiones para identificar anomalías regionales del hipocampo en la depresión de inicio precoz y en la de inicio tardío. También se investigaron los factores relacionados con la morfología del hipocampo. Método: Mediante resonancia magnética de alta resolución se comparó la morfología del hipocampo de pacientes ancianos con depresión de inicio precoz (n = 24) y de inicio tardío (n = 22) y la de individuos de comparación (n = 34). Se identificaron alteraciones estructurales mediante la comparación de distancias, medidas desde puntos homólogos de la superficie del hipocampo hasta el núcleo central del modelo de la superficie del hipocampo de cada individuo, entre grupos. Resultados: Los volúmenes del hipocampo difirieron entre los pacientes deprimidos y los individuos de comparación, pero no entre los pacientes con depresión de inicio precoz y los que tenían depresión de inicio tardío. No obstante, los resultados cartográficos estadísticos demostraron que las contracciones de la superficie regional fueron significativamente pronunciadas en la depresión de inicio tardío en comparación con las de la depresión de inicio precoz en la región anterior del subículo y posterolateral del subcampo CA1 en el hemisferio izquierdo. Se observaron diferencias significativas bilaterales en la forma de los subcampos CA1 a CA3 anteriores y del subículo entre los pacientes y los individuos de comparación. Estos resultados fueron similares cuando se comparó por separado cada grupo de enfermedad con los individuos de control. Las contracciones de la superficie del hipocampo se relacionaron significativamente con las medidas de memoria en los pacientes deprimidos con depresión de inicio tardío, pero no en los pacientes con depresión de inicio precoz o los individuos de comparación. Conclusiones: La presencia de déficit más acusados en el volumen regional y sus asociaciones con la memoria en la depresión de inicio tardío pueden sugerir que estos pacientes tienen una probabilidad mayor de desarrollar deterioro cognitivo a lo largo del tiempo que los individuos con depresión de inicio precoz. Las anomalías regionales del hipocampo en la cartografía y sus correlatos cognitivos pueden servir de ayuda para guiar la investigación en torno a los perfiles de riesgo y las estrategias de tratamiento.
