El trastorno límite de la personalidad en la práctica clínica.

- Abstract
La mayoría de los estudios del trastorno límite de la personalidad examinaron a pacientes procedentes del ámbito hospitalario o extrahospitalario. Los objetivos de este estudio consistieron en examinar las características de los pacientes con trastorno límite de la personalidad en la práctica clínica cotidiana y en utilizar los datos procedentes de una muestra comunitaria de pacientes con trastorno Iímite de la personalidad para perfeccionar el constructo "borderline. " Método: Una muestra nacional aleatoria de 117 psiquiatras y psicólogos experimentados, procedentes de las listas de afiliados de la American Psychiatric Association y de la American Psychological Association, ofreció información sobre un paciente con trastorno límite (n = 90) o distímico (n = 27) de la personalidad seleccionado, de forma aleatoria, de los pacientes de su consulta. Los clínicos ofrecieron información sobre la comorbilidad del Eje I, la comorbilidad del Eje II y el funcionamiento adaptativo, junto con una descripción de la personalidad del paciente utilizando el Shedler-Westen Assessment Procedure-200 (SWAP-200) Q-sort, un instrumento diseñado con fines evolutivos y taxonómicos. Los análisis compararon las variables de interés de los grupos con trastorno límite y distímico de la personalidad y agruparon los ítems del SWAP-200 de todos los pacientes con trastorno límite de la personalidad para elaborar un retrato global de dicho trastorno desde la perspectiva comunitaria. Resultados: La muestra de pacientes con trastorno límite de la personalidad guardo un gran parecido con las muestras del trastorno límite de la personalidad examinadas anteriormente en cuanto a comorbilidad y funcionamiento adaptativo. El SWAP-200 ofreció, sin embargo, una imagen de mayor sufrimiento y desregulación emocional de los pacientes con trastorno Iímite de la personalidad en comparación con la descripción del DSM-IV. Conclusiones: Los pacientes con trastorno límite de la personalidad de las muestras experimentales se parecen, considerablemente, a los observados en un corte transversal de la práctica clínica. Diversos estudios han reproducido, sin embargo, una imagen de los síntomas del trastorno Iímite de la personalidad que incide, en mayor medida que la descripción del DSM-IV, en el sufrimiento psicológico intenso de dichos pacientes, sugiriendo posibles criterios diagnósticos del DSM-V.
