SÍNDROME METABÓLICO Y RIESGO CARDIOVASCULAR EN PACIENTES CON OBESIDAD MÓRBIDA SOMETIDOS A BYPASS GÁSTRICO.

- Abstract
Objetivos: Los pacientes con obesidad mórbida (OM) presentan una elevada morbi-mortalidad cardiovascular (CV) debida a su asociación con varios componentes del Síndrome Metabólico (SM). El objetivo de este estudio ha sido determinar la prevalencia del SM y de otros factores de riesgo CV en pacientes con OM y analizar el efecto que el bypass gástrico ejerce sobre el riesgo CV a los 2 años de seguimiento. Material y métodos: Se ha evaluado a 46 pacientes con OM (80,4% mujeres) de 39,4 ± 10,5 años con un IMC de 45,1 ± 3,1 kg/m² a los que se les practicó un bypass gástrico laparoscópico. Se definió el SM según criterios diagnósticos de la IDF 2005. se analizó antes y a los 2 años tras la cirugía los siguientes parámetros: TA, glucosa, insulinemia, perfil lipídico, ácido úrico y resistencia insulínica (RI). La RI era determinada por hiperinsulinemia (insulina > 16,7 mU/l) e índice HOMA-IR (> 3,8). También se calculó el riesgo CV en 10 años según las puntuaciones de Framingham. Resultados: Previo a la cirugía, la prevalencia de SM fue del 60,9%, el 67,2% de los pacientes presentaban RI, el 43,5% HTA, el 34,8% Dislipemia, el 28,3% disglucemia y el 10,9% hiperuricemia. En el 21,7% de los pacientes se categorizó un nivel de riesgo CV en 10 años elevado. A los 2 años tras la cirugía, el IMC fue de 29,1 ± 4 kg/m² con una reducción ponderal total de 42,6 kg. Todos los pacientes normalizaron su insulinemia e índice HOMA y solo 1 enfermo siguió cumpliendo criterios diagnósticos de SM. El 92,3% de los pacientes con disglucemia, el 100% de los hiperuricémicos, el 93,8% de los dislipémicos y el 85% de los hipertensos normalizaron sus perfiles metabólicos. Todos los enfermos redujeron su riesgo CV inicial de un nivel elevado a un nivel bajo, excepto en un caso en que el riesgo CV siguió siendo alto. En la siguiente tabla se presenta un resumen de las variables estudiadas: Conclusión: En pacientes con OM, el bypass gástrico disminuye de forma significativa el riesgo CV y la prevalencia del SM y podría considerarse el tratamiento de elección y más efectivo a largo plazo en aquellos obesos mórbidos que presenten un riesgo CV elevado.
