- Abstract
Introducere. Protezele pe conducere osoasă, cum ar fi proteza BAHA, reprezintă o soluţie chirurgicală cu riscuri minime pentru pacienţii cu hipoacuzie de transmisie sau mixtă, cu o recuperare funcţională nesatisfăcătoare după chirurgia otitei sau care nu pot beneficia, din diverse motive, de chirurgie funcţională. Material şi metodă. în perioada 2008-2015, în Clinica ORL a Spitalului Clinic Judeţean de Urgenţă Cluj au beneficiat de proteză BAHA 32 de pacienţi. Bateria de teste audiologice efectuată preoperator a cuprins, după caz, audiograma tonală liminară, audiograma vocală, iar la copii, potenţiale evocate auditive şi ASSR. Postoperator evaluarea audiologică a cuprins audiograma tonală în câmp liber şi audiograma vocală. Pacienţii adulţi şi copiii mai mari au fost rugaţi să răspundă preoperator şi postoperator la chestionarul Glasgow Benefit Inventory pentru a evalua mpdificarea calităţii vieţii acestor pacienţi după implantare. Rezultate. în perioada 2008-2015, în Clinica ORL â Spitalului Clinic Judeţean de Urgenţă Cluj au beneficiat de proteză BAHA 32 de pacienţi, 26 fiind purtători de’implant BAHA, iar 6 copii purtători de cordeluţă BAHA. Pragurile auditive s-au îmbunătăţit pe cele 4 frecvenţe testate cu 28.65 până la 45.00 dB HL. Inteligibilitatea vorbirii a crescut cu 61.92% după activarea implantului BAHA. Complicaţiile postoperatorii au fost comparabile cu cele descrise în literatura de specialitate. Scorurile chestionarului GBI au arătat o îmbunătăţire semnificativă a calităţii vieţii pacienţilor ca urmare a utilizării protezei BAHA. Concluzii. Implantul BAHA reprezintă o soluţie funcţională cu riscuri chirurgicale minime şi cu prognostic bun dacă se respectă indicaţiile şi procedurile chirurgicale. Implantul BAHA îmbunătăţeşte pragul auditiv şi inteligibilitatea vorbirii, având un ecou favorabil asupra calităţii vieţii pacienţilor.
