- Abstract
Introducción. La infección por el virus de la hepatitis C (VHC) es un problema de salud pública. Actualmente se cuenta con antivirales de acción directa (AAD) que permiten el aclaramiento viral en 95 % de los casos. Objetivo. Caracterizar los genotipos virales y las sustituciones asociadas con la resistencia a los AAD (RAS) del VHC en personas con antecedente de transfusión antes de 1996 (PHT), en aquellos con infección por VHC sin tratamiento previo con AAD y en pacientes con hepatopatías terminales asociadas sometidos a trasplante hepático (TH). Materiales y métodos. Se amplificaron y secuenciaron las regiones del genoma viral: 5'UTR, NS5A y NS5B en 4, 3 y 13 muestras de los grupos de estudio, respectivamente. Luego de la edición, ensamblaje y alineamiento de las secuencias, se hizo un análisis filogenético para la caracterización de genotipos usando métodos de inferencia bayesiana y máxima verosimilitud mediante Mr Bayes y IQTREE. En la identificación de RAS se usaron alineamientos con base en la secuencia de referencia (GenBank NC_004102). Resultados. El genotipo 1 del VHC se caracterizó en 16 muestras y los subgenotipos 1a y 1b en 12 y 4 muestras, respectivamente, y el genotipo 2, el subgenotipo 2a, y el genotipo 4, subgenotipos 4a y 4d, en una muestra cada uno. Se identificaron tres RAS: Q30R, C316N y Y93H en 2 muestras de PHT y en 6 muestras de TH. Conclusiones. El genotipo 1 de VHC, subgenotipo 1b, sigue siendo prevalente en el país, independientemente del factor de riesgo. Se hizo la primera caracterización de los subgenotipos 4a y 4d y la primera identificación de RAS en Colombia.
