- Abstract
Objetivo. Investigar la seroprevalencia de anticuerpos contra el SARS-CoV-2 en la población colombiana para comprender la dinámica de transmisión del virus en el país, así como estimar las tasas de ataque y determinar los factores de riesgo asociados a la transmisión del virus. Materiales y métodos. Se diseñó un estudio transversal de base poblacional para evaluar la seroprevalencia de anticuerpos contra el SARS-CoV-2 en once ciudades de Colombia entre septiembre de 2020 y junio de 2021. El estudio involucró un muestreo por conglomerados de múltiples etapas en cada ciudad. Los participantes proporcionaron una muestra de suero y respondieron un cuestionario demográfico y de factores de riesgo. La infección previa por SARS-CoV-2 se determinó usando el ensayo de quimioluminiscencia SARS-CoV-2 Total (COV2T) Advia Centaur - Siemens. El análisis estadístico incluyó un método bayesiano para ajustar la estimación de la seroprevalencia teniendo en cuenta la incertidumbre de la sensibilidad y la especificidad del estudio previo de validación. Para identificar la asociación entre la seroprevalencia y las variables independientes se utilizó un modelo lineal generalizado de efectos aleatorios. El estudio fue aprobado por el Comité de Ética del Instituto Nacional de Salud (CEMIN 010/2020). Se obtuvo el consentimiento informado por escrito de cada participante adulto, así como el consentimiento oral y el permiso de los padres por escrito de todos los participantes entre los cinco y los 17 años de edad. Resultados. Un total de 18.804 participantes de 7.495 hogares participó en el estudio. La seroprevalencia varió entre las ciudades, desde el 21 % (IC95%: 16-25 %) en Medellín al 78 % (IC95%: 65-91 %) en Guapi. Se encontró que la seropositividad fue menor en adultos de 60 años o más y mayor en población afrodescendiente. Asimismo, se encontró que los participantes de los estratos socioeconómicos más bajos presentaron una mayor proporción de seroprevalencia comparados con los de estratos altos. Conclusiones. Colombia ha sido uno de los países latinoamericanos más afectados por la pandemia de COVID-19. Este estudio documentó tasas de ataque muy altas en varias ciudades colombianas hacia fines del 2020 y determinó los impulsores claves de la heterogeneidad, incluida la etnia y el estrato socioeconómico. Se han hecho pocos estudios de seroprevalencia del SARS-CoV-2 en Latinoamérica. Este contribuye a la comprensión de la dinámica de la transmisión del virus en el país.
