- Abstract
Fumar es una enfermedad adictiva crónica responsable de la muerte de más de 7 millones de personas cada año. Aunque la relación directa entre el hábito de fumar y el daño renal no está bien establecida, existen numerosos compuestos en el tabaco que son capaces de causar nefropatías. Los objetivos de este trabajo fueron dilucidar si el consumo de tabaco promueve la excreción de alguno de los biomarcadores urinarios más conocidos de daño renal temprano [proteinuria, molécula de daño renal-1 (KIM-1), N-acetil-beta-D-glucosaminidasa (NAG) y lipocalina asociada a gelatinasa de neutrófilos (NGAL)]; y evaluar la existencia de una correlación entre estos biomarcadores y el nivel de exposición al tabaco, medido en términos de cotinina urinaria (metabolito de la nicotina). Se recogieron muestras de orina de voluntarios de la consulta de Atención Primaria de un Centro de Salud (Cáceres), sin daño renal previo ni factores de riesgo de enfermedad renal. Éstos se dividieron en dos grupos: fumadores (n=56) y no fumadores (n=147). Se creó una base de datos con sus principales parámetros antropométricos, así como sus niveles de creatinina plasmática (marcador estándar de daño renal). Se cuantificaron los niveles urinarios de proteínas, NAG, NGAL, KIM-1 y cotinina. Posteriormente, cada biomarcador se correlacionó con la cotinina. No se observaron diferencias significativas en las principales variables antropométricas consideradas. Tampoco se encontraron diferencias en relación con sus niveles de creatinina plasmática. Por el contrario, se detectaron concentraciones urinarias significativamente más altas de KIM-1, proteinuria y NGAL en el grupo de fumadores. Además, estos biomarcadores mostraron una correlación directa significativa con los niveles urinarios de cotinina. Nuestros resultados sugieren que en los pacientes fumadores se han desencadenado procesos patológicos que han favorecido la excreción de algunos biomarcadores ampliamente conocidos. En conclusión, parece existir una relación entre fumar y el desarrollo de daño renal temprano.
