- Abstract
Peripartum hysterectomy at the time of caesarean delivery or afterwards for puerperal sepsis with necrotic uterus is the unfortunate result of prolonged obstructed labour with infection. There has never been a trial comparing instrumental delivery with caesarean in prolonged obstructed labour and there likely never will be, so fully understanding the adverse sequelae of attempting instrumental delivery in this setting when compared with caesarean delivery is impossible. One can also safely say that a destructive procedure for stillbirth in the setting of prolonged obstructed labour would be safer than a caesarean delivery if, I and only if i , performed by an experienced provider with adequate anaesthesia and proper instrumentation.
