- Abstract
Cough is a common symptom which makes the patient to address for medical attention when it no longer improves and becomes annoying. Cough lasting more than 8 weeks is evaluated as chronic, refractory, idiopathic or unexplained, that does not respond to classic antitussive preparations and requires a differentiated and thorough approach. Cough and non-cough stimuli, including subtreshold, through a series of biological peptides, mediators of inflammation and immune processes, can generate a series of nonspecific mechanisms. In these cases, new strategies are needed based on the elucidation of peripheral and central mechanisms of hypersensitivity to cough. Research on centrally acting neuromodulators – GABA agonists (baclofen), antidepressants (amitriptyline), antiepileptics (gabepentin, pregabalin), potential transient receptor channel antagonists, new tachykin antagonists, P2X3 purinergic receptor blockers (gefapixant), H1-antihistamines and opioids – opens new perspectives in the pharmacotherapy of chronic refractory cough.
