Neuropsiquiatría de las infecciones cerebrales: ¿en qué pacientes debe realizarse una punción lumbar?

- Abstract
Antecedentes: las infecciones cerebrales agudas, y en particular la encefalitis viral, provocan múltiples alteraciones psiquiátricas. El diagnóstico confirmatorio requiere la realización de un estudio invasivo y doloroso; la punción lumbar. No existen criterios claros en la literatura médica para indicar una punción lumbar en pacientes psiquiátricos, y esto repercute en errores comunes de diagnóstico, de acuerdo con un estudio previo realizado en el INNN. Objetivo: diseñar una escala clínica para identificar casos de encefalitis viral u otras enfermedades infecciosas en pacientes con alteraciones neuropsiquiátricas agudas. Material y métodos: 1. Estudiamos 83 casos de encefalitis aguda de probable etiología viral. Registramos sus signos y síntomas más frecuentes, los cuales fueron buscados ulteriormente en 104 casos de psicosis aguda, de primer episodio y naturaleza no infecciosa: trastornos del espectro de la esquizofrenia (80.7%), trastornos afectivos y otros diagnósticos (20.3%) 2. Tras realizar un análisis comparativo, con el uso de pruebas de chi-cuadrada y razones de verosimilitud (likelihood ratios, LR), se construyó una escala ordinal con los signos/síntomas que discriminaron adecuadamente entre ambos grupos. Se asignó un valor diagnóstico a cada dato clínico, de acuerdo con el likelihood ratío previamente calculado. 3. Tras calificar cada caso de acuerdo a la escala, determinamos su sensibilidad y especificidad, con diferentes puntos de corte, mediante el uso de una curva de ROC. Se realizaron ejercicios de confiabilidad con una muestra aleatoria (n=30). Se han integrado otros grupos de casos para confirmar la validez discriminativa de la escala: neuroinfección bacteriana (n= 69), y meningoencefalitis tuberculosa (n=53). Resultados: la comparación entre pacientes con encefalitis aguda y pacientes psiquiátricos de naturaleza no infecciosa, no mostró diferencias significativas en cuanto a edad y género, pero sí identificó 12 signos o síntomas que orientan a la presencia de una encefalitis viral (tabla 1). Éstos constituyen los elementos de la escala clínica propuesta en la tabla 2. Con una calificación de 3 puntos, se alcanza una sensibilidad de 95% y una especificidad de 80%. Esto se ha confirmado mediante la comparación con los otros grupos de enfermedades infecciosas. Discusión: empleo de un enfoque sistematizado (por ejemplo la escala aquí propuesta) podría ayudar a formular criterios clínicos para la detección de infecciones cerebrales, y a reducir la frecuencia de errores de diagnóstico durante el abordaje de pacientes con alteraciones neuropsiquiátricas agudas. En el escenario clínico una escala de esta naturaleza podría usarse como un tamizaje clínico, es decir, como una herramienta semiológica de aplicación rápida para fortalecer la toma de decisiones, por ejemplo la indicación de una punción lumbar.
