- Abstract
Background: The paper investigates the risk factors for mortality in patients with ST-segment elevation myocardial infarction (STEMI) subjected to percutaneous coronary intervention (PCI) hospitalized at the department of acute coronary syndromes (ACS). Material and Methods: The records of 508 patients with STEMI treated with PCI at the ACS department from 2009-2013 in Rzeszow, Poland were retrospectively analyzed. Research tool was a survey questionnaire. Results: Higher mortality in patients without hypertension prior to infarction (16%) and higher risk in patients with prehospital sudden cardiac arrest (SCA) (43.9%) were observed. Cardiogenic shock (68.3%) and prior interventions in coronary arteries (17%) increased the death risk. Waiting time for contact with medical staff increased mortality. The patients who died waited approx. 6 hours, while the survivors almost twice less. Serum glucose level was significantly higher among patients who died. Fewer deaths were observed in the smokers (3.7% smokers vs. 11.6% non-smokers). Conclusions: Prehospital episode of SCA and cardiogenic shock, untreated hypertension, negative cigarette smoking history, infarction in the lateral or anterio-lateral wall and previous PCI increase the mortality. Mortality increases in the patients waiting longer for medical care. Elevated serum glucose level increased the risk of death.
