- Abstract
Objetivo: Los hijos de padres con trastorno depresivo mayor tienen un riesgo de depresión mayor tres veces superior al de los individuos sin estos antecedentes familiares. Aunque la depresión mayor es una causa significativa de morbilidad y mortalidad, los correlatos neurales del riesgo de depresión mayor todavía son poco comprendidos. Este estudio compara la activación de la amígdala y del núcleo accumbens en niños y adolescentes con alto y bajo riesgo de depresión mayor en diferentes condiciones de atención y emocionales. Método: Treinta y nueve jóvenes, 17 de ellos descendientes de padres con depresión mayor (grupo de alto riesgo) y 22 descendientes de padres sin antecedentes de depresión mayor, ansiedad ni trastornos psicóticos (grupo de bajo riesgo), completaron un estudio por resonancia magnética funcional. Durante la obtención de las imágenes, los individuos contemplaron rostros con expresiones emocionales de intensidad variable, separados en bloques en los que la atención no estaba restringida (contemplación pasiva) y bloques en los que la atención estaba restringida (evaluar la amplitud de la nariz del rostro, evaluar el propio temor subjetivo ante la contemplación del rostro). Resultados: Cuando la atención no estaba restringida, los individuos del grupo de alto riesgo mostraron un mayor grado de activación de la amígdala y del núcleo accumbens ante los rostros temerosos y un menor grado de activación del núcleo accumbens ante los rostros felices (volumen pequeño corregido para la amígdala y el núcleo accumbens). Durante la condición de restricción de la atención no aparecieron diferencias entre los grupos en cuanto a la activación de la amígdala o del núcleo accumbens. El análisis exploratorio mostró que la restricción de la atención se asociaba con un grado de activación de la corteza prefrontal medial mayor en el grupo de alto riesgo que en el de bajo riesgo. Conclusiones: Las respuestas de la amígdala y del núcleo accumbens frente a los estímulos afectivos podrían reflejar una vulnerabilidad a la depresión mayor. La restricción de la atención podría normalizar la función neural relacionada con la emoción, posiblemente a través de la participación de la corteza prefrontal medial; la contemplación de rostros sin restringir la atención podría comportar procesos aberrantes asociados con el riesgo de depresión mayor.
