- Abstract
Peripheral arterial disease is a marker for generalized atherosclerosis. Endothelial dysfunction plays a key role in the development, progression and clinical manifestations of atherosclerotic disease. Th e presence of coronary and peripheral arterial disease concurrently has a huge negative impact on cardiovascular morbidity and mortality. Risk stratification before major vascular surgery is an everyday challenge for the clinical cardiologist. Th e prediction of events in this set of patients bears important implications epidemiological, clinical and practical. Cardiovascular complications account for approximately half of all mortality after non cardiac surgery and are the leading cause of death in those patients. When both severe and symptomatic peripheral vascular diseases were present, the risk of death due to coronary artery disease was 10 to 15 times higher. Th e need of effective risk stratification is to select patients in order to face safely the surgical procedure, by balancing the benefi t of each procedure with the inherent risks.
