- Abstract
L’esecuzione di blocchi nervosi periferici a scopo diagnostico è una metodica che consiste nell’applicazione di un anestetico locale (es. lidocaina) per via percutanea in adiacenza di un tronco nervoso periferico (motorio o misto) con conseguente rapida e temporanea insorgenza di ipo/anestesia del dermatomero corrispondente e/o paresi della muscolatura di pertinenza. Per identificare il ramo nervoso target una delle tecniche maggiormente utilizzate è quella che prevede l’ausilio dell’ultrasonografia come strumento di assistenza e guida. La paresi temporanea del distretto muscolare di pertinenza del tronco nervoso oggetto di blocco diagnostico risulta utile nell’ambito del management del paziente affetto da spasticità al fine di dirimere l’ipertonia spastica dalle altre cause che possono spiegare il pattern obiettivato nel paziente. La letteratura scientifica sull’utilizzo della guida ecografica nell’esecuzione di blocchi diagnostici periferici nel setting della Medicina Fisica e Riabilitativa è sempre più ampia e concorde sul fatto che l’ausilio dell’ultrasonografia dia il vantaggio di poter avere una visualizzazione diretta dell’obiettivo, di poter verificare il corretto posizionamento dell’ago prima dell’inoculazione del farmaco oltre a poter visualizzare le strutture anatomiche adiacenti al nostro target nell’ottica di scegliere la via di accesso più sicura. Ad oggi tuttavia nonostante vastamente applicata vi sono solo iniziali evidenze sul fatto che la metodica eco-assistita determini dei benefici in termini di efficacia, tolleranza e riduzione dei tempi della procedura, rispetto all’esecuzione assistita da altre metodiche (es. stimolazione nervosa periferica).
