Interleukin-10 Promoter Polymorphisms and Multiple Sclerosis.

- Abstract
Multiple sclerosis is a chronic inflammatory disease of the central nervous system with an incidence of 1: 1000 individuals of caucasian origin. The disease results in destruction of myelin sheaths. This leads to the formation of demyelinated lesions in brain and spinal cord white matter, which is associated with impaired nerve conduction and to some extend axonal loss resulting in neurological disability. It has become increasingly evident that cytokines play a key role in the pathogenesis of MS. The balance between anti-inflammatory and pro-inflammatory cytokines might determine whether the immune response in MS is harmful or not. Interleukin-10 (IL-10) is an important anti-inflammatory cytokine, that inhibits the synthesis of proinflammatory cytokines. Increased levels of IL-10 mRNA is associated with stable disease activity, and decreased IL-10 mRNA levels has been detected in patients with increased disease activity, suggesting that IL-10 plays an important role in the control of MS. Characterisation of the IL-10 promoter region has revealed three point mutations at positions -1082, -819 and -592 from the transcription start site. The -1082 A allele has been associated with low and the -1082 G allele with high in vitro IL-10 production. We wanted to see if MS development and progress could be related to this polymorphism. 168 MS patients (59 men and 109 women) and 87 controls were typed for IL-10 promoter polymorphism. Of the 168 patients 135 were diagnosed with Relapsing Remitting Multiple Sclerosis, and 33 with Primary Progress Multiple Sclerosis. The patients had an average disease onset of 32.5±0.6 years ±SE, and average disease duration of 14.4±0.2 years ±SE. Life table analysis with Expanded Disability Status Scale (EDSS) ≥6 as endpoint stratified for IL-10 polymorfisms were performed. No significant correlation to IL-10 promoter polymorphism were observed for neither MS susceptibility or disease severity.
