- Abstract
Introducción. La malaria complicada se caracteriza por la presencia de signos, síntomas o alteraciones en pruebas diagnósticas que indican alguna disfunción de los órganos vitales. Esto puede producir daño y muerte celular en los diferentes órganos, además de grave extravasación de plasma que lleva al paciente a choque, hipoxia celular e inducción de metabolismo anaerobio, como resultado del compromiso intenso de los diferentes órganos y del sistema. Objetivo. Predecir el riesgo de malaria complicada en comunidades de los resguardos embera en Pueblo Rico, Risaralda. Materiales y métodos. El Sistema Nacional de Vigilancia en Salud Pública se creó para realizar de forma sistemática y oportuna el reporte sobre la dinámica de los eventos que puedan afectar la salud de la población y, con ello, tomar decisiones para la prevención y el control de enfermedades, determinar factores de riesgo en salud y optimizar el seguimiento y la evaluación de las intervenciones. Se tomó la base de datos de los eventos de malaria reportados en el Sistema Nacional de Vigilancia en Salud Pública y se exporto al software R, donde se construyó un modelo predictivo para establecer el riesgo de malaria complicada mediante machine learning con un modelo de Random Ferns. Se definió que el modelo estaría integrado por las variables: sexo, área, “tip_ss_”, “estrato_”, “recrudece”, “esp_pla”, “gametocito”, “desplazami”, etnia, edad, recuento, “complicaci”, “clas_caso” y “sintomatic”. Estas variables se establecieron como un marco de datos para que el programa R pudiera procesar y desarrollar códigos que definieran un modelo predictivo donde el resultado correspondería a la variable malaria complicada determinada como: “complicaci” Resultados. Después de obtener el modelo Random Ferns, se evaluó el modelo con datos nuevos y se encontró que este carecía de falsos negativos, dada su gran sensibilidad para la predicción de casos de malaria complicada. Con este modelo, se reportó un 19,5 % de falsos positivos, para una especificidad del 80 %. Finalmente, la exactitud del modelo estuvo entre el 75 y el 86 %. Se anota este rendimiento del modelo de Random Ferns con una prevalencia de complicación del 2 %. Conclusiones. El modelo predictivo de Random Ferns fue la fuente para el desarrollo de una aplicación virtual en R que ayudará a identificar el riesgo de malaria complicada. El modelo predictivo de malaria complicada puede ser utilizado por personal auxiliar o técnico en una intervención comunitaria, en la cual los gestores de salud identifiquen los sujetos de monitoreo y traten los casos de malaria. De esta forma, las comunidades se pueden apoyar para la adecuada gestión del riesgo por malaria.
