Fire and explosion at Synthron.
- Authors
Patterson, Ken
- Abstract
This incident occurred because changes to personnel and operating practice were introduced in an uncontrolled way on a site with lax, or non-existent, management of change procedures. Combined with other process safety failings, these changes gave rise to a devastating fire and explosion. The accident left one employee dead and injured a further 14, destroyed the factory and damaged buildings in the surrounding community. This summary of the event is drawn from the US Chemical Safety Board investigation report.
