- Abstract
The article presents research findings and recommendations for the hand-off of discharged patient data from hospital-based physicians to primary care physicians. Common deficits in communication and information transfer included the availability, timeliness, content, and format of discharge communications, as well as primary care physician satisfaction. Measures of these deficits showed the communication problems to be common and with adverse effects on patient care. Recommendations include provision of computer-generated summaries of pertinent patient data and use of standardized formats for data presentation to primary care physicians.
