- Abstract
Objetivo: estimar la prevalencia de soledad y aislamiento social, así como su asociación con la incidencia de discapacidad, demencia, y mortalidad en adultos mayores mexicanos de población abierta en áreas de ingreso económico medio- bajo, tras tres años de seguimiento. Antecedentes: la soledad se define como el sentimiento de estar solo, separado o apartado de otras personas, se puede considerar como persistente, inevitable y displacentero. El aislamiento social se caracteriza por la disminución en la frecuencia de contacto de la red social que puede incluir o no miembros de la familia. El estudio de cada una de estas entidades es importante debido a su alta frecuencia en las PAM y su asociación con el desarrollo de discapacidad, demencia e incluso mortalidad. Métodos: se realizó un análisis secundario del seguimiento a tres años de 1,668 adultos de 65 años o más (PAM), evaluadas dentro de los protocolos del Grupo de Investigación en Demencias 10/66 en México. La presencia de soledad se evaluó con tres preguntas incluidas en el Inventario del Estado Mental Geriátrico (GMS): ¿se siente solo?, ¿este sentimiento de soledad le genera malestar o tristeza?, ¿puede evitar sentirse solo? El aislamiento social se identificó mediante la evaluación de los contactos con amigos, familiares, vecinos, la corresidencia; así como la asistencia a la iglesia y a reuniones sociales. Se estimo la asociación de la soledad con y sin malestar emocional, así como del aislamiento social con la incidencia de discapacidad, demencia, y mortalidad tras tres años de seguimiento, mediante modelos multivariados de riesgos por competencias. Núm. de registro del protocolo: 40/16 Resultados: se observó una distribución diferencial de la soledad con y sin malestar emocional, los grupos de edad y la escolaridad por área de estudio (Cuadro 1). Tras tres años de seguimiento la soledad se encontró asociada a demencia incidente subHR=1.5 (IC95%1.1-2.1), y el aislamiento social a la mortalidad, subHR=1.5 (IC95%1.1-2.0). Conclusiones: los antecedentes del estudio del sentimiento de soledad y del aislamiento social en las PAM presentan diversos retos entre los que se encuentran la dificultad del reporte de cada una de estos, ya que algunas escalas evalúan ambas entidades e incluso su percepción de apoyo recibido. Los resultados de este reporte parecen evidenciar que el efecto de dichas condiciones es diferencial en cuanto al desarrollo de demencia y mortalidad en las PAM.
