Caracterización de los perfiles inmunológicos en un modelo murino de hipersensibilidad a los antígenos de Mycobacterium tuberculosis tratada con fármacos antituberculosos.

- Abstract
Objetivo: determinar el papel etiopatogénico de los neoantígenos de pared celular y citoplasma derivados de la lisis celular tras la antibioterapia de Mtb en la hipersensibilidad debida al tratamiento antituberculoso en un modelo murino. Antecedentes: la TB es una enfermedad infecciosa crónica causada por Mtb con afección pulmonar, cerebral y sistémica. Su tratamiento logra tasas de recuperación elevadas, sin embargo, 5 al 10% de los pacientes cursa con un período de mejoría seguido de agravamiento de las manifestaciones clínicas y radiológicas, fenómeno denominado respuesta paradójica al tratamiento antituberculoso, cuyo origen inmunopatológico permanece elusivo, aunque se postula que el estrés selectivo de los antibióticos modifica el fenotipo de la pared celular y el citoplasma, generando neoantígenos que inducen una respuesta inmune exacerbada. Métodos: se desarrolló un modelo murino longitudinal, prospectivo y descriptivo de hipersensibilidad IV a la administración de fracciones proteicas de pared celular y citoplasma de Mtb cultivada bajo estrés farmacológico in vitro con INH, RIF y EMB; evaluando la cinética de las alteraciones histológicas, inmunohistoquímicas, celulares y conductuales. Resultados: los ratones expuestos a pared celular tuvieron un deterioro conductual mayor, mientras que los ratones expuestos a citoplasma tuvieron una ganancia ponderal significativamente menor en comparación con los otros grupos. La histología e inmunohistoquímica mostraron gran heterogeneidad de lesiones en los órganos evaluados, tales como granulomas y vasculitis en hígado, cerebro; así como neumonía e hiperplasia linfoide en pulmones. Esto es característico de un proceso inflamatorio aséptico inducido por componentes subcelulares sin involucro de Mtb metabólicamente viable. Finalmente, el análisis de varianza de las concentraciones séricas de IL-4 e IFN-γ determinó que no existen diferencias estadísticamente significativas en las concentraciones séricas de los grupos. Conclusiones: los Ag generados por Mtb durante su desarrollo bajo estrés farmacológico son capaces de generar alteraciones morfológicas, inmunes y conductuales sin la necesidad de la presencia de la micobacteria viva, sin embargo, las alteraciones observadas no difieren tras el análisis estadístico en relación con las fracciones antigénicas inoculadas en los animales de este modelo experimental de hipersensibilidad a antígenos.
