- Abstract
Las alteraciones de la marcha de etapas tardías o tempranas de la enfermedad de Parkinson (EP) no responden a tratamiento dopaminérgico. La banda sin fin con soporte parcial de peso (SPP), produce reestructuración de la marcha. Ventajas de la estimulación sensorial auditiva: da estabilidad temporal a los movimientos, mejora los bloqueos y la depresión. Objetivos: valorar la eficacia de la estimulación sensorial auditiva en la reeducación de la marcha en los pacientes con EP. Material y métodos: ensayo clínico controlado simple ciego. Realizado de junio a noviembre de 2006. Se aleatorizaron 39 pacientes, en 2 grupos. Criterios de inclusión: diagnóstico de enfermedad de Parkinson idiopática, evolución de enfermedad entre 5 a 10 años, Hoehn-Yahr 2 a 3, respuesta a levodopa. Se hizo la valoración basal de la marcha de ambos grupos. Durante el programa de rehabilitación de la marcha en la banda sin fin se dieron instrucciones verbales para reeducar la marcha; al grupo A se le aplicó un estímulo sensorial auditivo rítmico, incrustado en música de tipo ambiental, ejecutada con sintetizador, a un ritmo de 120 beats/minuto, compás de 4/4 y, al grupo B, no se le aplicó esta técnica. Se aplicaron 6 semanas de tratamiento (3 sesiones/semana, cada sesión duró 20 minutos). Finalmente se hizo la valoración final de la marcha y el equilibrio. Resultados: resultados: valoración inicial: hubo diferencias significativas entre ambos grupos (el grupo A se encontraba en un estado de enfermedad más avanzado que el grupo B). Valoración postratamiento: los 39 pacientes mejoraron significativamente la velocidad de la marcha y presentaron reducción importante del tiempo cronometrado en 10 m planos (TWT). Mejoraron las puntuaciones de UPDRS, H-Y, S&E, escala de la marcha de MM (valor de p = 0.000) y hubo leve mejoría para las escalas de Berg e Inventario Beck (p > 0,05). El análisis de mediciones repetidas no mostró diferencias significativas entre ambos grupos. Conclusiones: este es un estudio piloto negativo, comprueba que la rehabilitación de la marcha en la EP, es muy benéfica para los pacientes. Metodológicamente la comparación de las características entre ambos grupos no es factible. La estimulación auditiva no ofreció ningún beneficio extra a los pacientes que la recibieron, aunque existen tendencias estadísticas en la mejoría de bradicinesia de la marcha y en la disminución de caídas.
