Carga de trabalho de enfermagem como fator relacionado a ocorrência de lesão por pressão em pacientes críticos: um estudo prospectivo.

- Abstract
Objetivo: Verificar a associação entre a ocorrência de lesão por pressão (LP) com variáveis demográficas e carga de trabalho de enfermagem na Unidade de Terapia Intensiva (UTI). Métodos: Estudo de coorte prospectivo realizado em pacientes da UTI de um hospital universitário, avaliados diariamente quanto ao desenvolvimento de LP. Utilizouse do teste de Wilcoxon para verificar a associação entre as variáveis quantitativas e o teste exato de Fisher para as qualitativas. Resultados: Foram incluídos 50 pacientes, sendo que aqueles com ausência de lesão (n=33) eram predominantemente do sexo feminino (70%), idade inferior a 60 anos (64%), brancos (70%), IMC maior que 24,9 (61%), tempo mediano de 5 dias de internação e admitidos devido à necessidade de acompanhamento clínico (39%). Os valores medianos dos escores SAPS 3 (escore de gravidade), Braden (escore de risco de LP) e NAS (escore de carga de trabalho de enfermagem) foram 51, 15 e 86 pontos, respectivamente. Os pacientes que desenvolveram LP (n=17) eram do sexo feminino (59%), idade inferior a 60 anos (53%), brancos (70,5%), IMC maior que 24,9 (70,5%), tempo mediano de internação de 6 dias e admitidos na UTI por instabilidade hemodinâmica (59%). Os valores medianos do SAPS 3, Braden e NAS foram 78, 11 e 96 pontos. As variáveis SAPS 3, Braden e carga de trabalho foram associadas à ocorrência de LP ambas com p<0,001. Conclusão: A alta carga de trabalho pode atuar como um fator associado a ocorrência de LP.
