- Abstract
This paper provides a concise review of the key features of cerebellar strokes with a particular consideration of vestibular and audiological dysfunctions. Acute vertigo is the most common neurootological symptom of cerebellar stroke. Although vertigo ascribed to cerebellar stroke is usually associated with other neurological "central" symptoms or signs, it may also sometimes mimic acute peripherial vestibulopathy, for example in cerebellar infarction in the territory vascularized by the medial branch of the posterior inferior cerebellar artery (PICA). Cerebellar infarction in the territory usually vascularized by the anterior inferior cerebellar artery (AICA) can produce unilateral hearing loss, a unique syndrome of stroke that it is typically accompanied by other, usually more prominent symptoms. The low incidence of vertigo associated with infarction involving the superior cerebellar artery (SCA) vascularization area may be useful to distinguish it clinically from PICA or AICA-related infarctions. This paper underlines the importance of audiovestibular symptoms in the diagnostic process in patients with cerebellar strokes and great usability of head impulse test in differentiation of acute vestibulopathy from central vestibular dysfunction.
