- Abstract
Introduction: Radiofrequency ablation of cavotricuspid isthmus is a useful technique for the treatment of typical atrial flutter. Entrainment mapping represents a valuable method for localizing reentry circuits. In typical atrial flutter, entrainment results in postpacing intervals (PPI) close to the tachycardia cycle length (TCL). Antiarrhythmic drugs influence conduction in atrial structures. The aim of our study was to asses the value of entrainment mapping in the presence of amiodarone in patients undergoing radiofrequency ablation (RFA) of isthmus-dependent atrial flutter. Methods and results: The study consisted of 324 patients with isthmusdependent atrial flutter: 204 were receiving amiodarone at the time of the study (group 1) and 120 were in a drug-free state (group 2). Entrainment was performed from lateral right atrial wall, cavotricuspid isthmus, interatrial septum and TCL, PPI and PPI minus TCL were determined. TCL was significantly longer in group 1 than in group 2 (249,36±33,55msec vs 238,27±18,52msec, p=0,0001) and PPI-TCL was significantly longer in group 1 than in group 2 in all regions: lateral right atrium (28,68±20,50 vs 7,69±7,31msec, p<0,001), isthmus (30,87±22,72 vs 8,58±9,97, p<0,001), interatrial septum (27,86±22,83 vs 16,38±29,89msec, p<0,001). More patients in group 1 had PPI-TCL>20 msec compared to group 2 (51,47% vs 8,33%, p<0,001). Conclusion: The presence of amiodarone changes the entrainment mapping data. In this setting a PPI exceeding the TCL by >20 msec doesn't exclude a typical form of atrial flutter and, therefore, neither the possible curative effect of radiofrequency ablation.
